Targeted Case Management
Eligibility:
- Adults with serious mental health and/or behavioral health
diagnosis.
- Adults with co-occurring conditions such as substance abuse
Goals:
- To assist the consumer in assessing circumstances and accessing
appropriate resources
- To coordinate the delivery of services across systems
Services:
- Intake assessment of adult and family needs
- Linkage to community resources
- Monitoring or services received
