Plus - Mental Illness/Chemical Abuse Program
Eligibility:
- 30 adults with a history of substance abuse and serious/persistent
mental illness
Goals:
- To access status of co-occurring disorders of mental illness
and substance abuse
- To make appropriate referrals for mental health and substance
treatment intervention
Services:
- Varied therapies (individual, couples, family and group)
- Case Management
- Resource referral and linkage
- Psychiatric evaluations and medication management available

Hours:
- Monday thru Friday – 9:00 am to 5:00 pm
