Children's
Targeted Case Management
Eligibility:
- Children and youth with serious mental health and/or behavioral
health diagnosis.
- Service extends to age 21 with no mental retardation diagnosis.
Goals:
- To assist the consumer in assessing circumstances and accessing
appropriate resources.
- To coordinate the delivery of services across systems.
Services:
- Intake assessment of child/youth and their family’s needs
- Linkage to community resources
- Monitoring or services received
Location:
Hours:
- On-call service is provided to ensure 24-hour emergency care.
